A pre-application meeting is held and a Shared Lives Officer will be allocated to you. They will support you throughout the process.

Application form is completed.

As part of the process we will complete a DBS check and undertake references in respect of you such as GP, employment, and mortgage.

You will need to attend your ‘Skills for Care’ training.

Your Shared Lives Officer will visit you around 8-10 times to complete your assessment.

This process takes between 3 and 6 months.

Your assessment will be shared with you when completed.

You will attend a regional approval panel where your Shared Lives Officer will present your assessment.

Your allocated Shared Lives Officer will continue to provide you with on-going support, advice and guidance.
