I am or will be going into hospital soon, if I need help when I leave, what happens?
You will not leave the hospital until you are well enough. The decision about when you are medically fit to leave will be made by your hospital doctor. Most people will not need additional care and support when they leave hospital but if you do, the ward will contact the discharge team to assess what help you might need. A multi-disciplinary team will make the decision about when you can leave when any support you need is in place. If you’re worried that you, a family member or friend might need help and you’re not sure that it’s been arranged, speak to staff on the ward.
I already have a care and support package, what will happen while I’m in hospital and when I leave hospital?
If you are only in hospital for a short time and your needs haven’t changed following your stay in hospital, it is likely that you will continue to receive the same support when you leave. If you are in hospital for an extended period, or your needs change, then your situation will be reassessed to work out what support you might need.

What if I find it difficult to cope when I get home?
If you return home from hospital and are finding it difficult to cope, contact Adult Social Care Direct on 024 7683 3003 or your named worker if you have one, to talk about what support might be available to you.
