Why we collect and use personal information about you

We collect and hold personal information relating to your Role as an Approved Shared Lives Carer / Approved Shared Lives Support Carer.

We use this information to:

  • Gain Approval Status with the Shared Lives Scheme
  • Provide you with necessary support and guidance for your role.
  • Help us to develop and improve our services
  • Complete statistical returns to Government departments and Care Quality Commission. (CQC)
  • CQC Inspections
  • Secure and Match Appropriate Placements
  • Manage, Review and monitor Placements.

We process personal data because it is necessary to comply with legal requirements. We do so to comply and enable us to perform tasks that evidence we have worked in the public’s best interest.

Who we share your personal information with

Personal information stored by the Shared Lives Scheme will only be shared on a need to know basis, when deemed necessary in the best interest of the person you support, the service we provide, and support offered to you as a Shared Lives Carer.

We will share data with:

  • Shared Lives Scheme staff.
  • West Midlands Shared Lives Consortium – For Panel purpose.
  • Coventry City Council Adult Social Care Teams – If Appropriate
  • Warwickshire County Council (WCC) – Adult Social Care Teams – If appropriate
  • Solihull Metropolitan Borough Council (SMBC) – Adults Social Care Team – If appropriate.
  • Financial Operation Teams – Across CCC WCC and SMBC, the Scheme will only share your personal data with the Local Authority that is responsible for payments to you.
  • Care Quality Commission.
  • Disclosure and Barring Service
  • Social Care Development Centre – Regarding your Training requirements.
  • Age UK/ Penderels/ Independent Advocates} if acting as Advocate or Appointee on behalf of a placement you support. 
  • Health and Social Care professionals ie GP/Community Nurses/ Consultants/ Dentists/Occupational Therapists/ Physiotherapists/Social Workers that maybe supporting a placement.  
  • Other Shared Lives Carers for the purpose of Respite placements.
  • West Midlands Fire Brigade for the purpose of Health and Safety Fire Safety Inspections.
  •  Service Users and families –for Introductions and matching purpose.   

The Shared Lives Scheme like to promote the Scheme’s service within CCC WCC and SMBC. We use CCC Website to promote the Scheme, sometimes with the use of Photography and Stories. We will seek consent individually prior to using any personal information or photographs. There will be times the Scheme wishes to use photography and personal stories externally, prior consent will be sought if sharing of personal data is used for this purpose.

How long will we keep your information?

Coventry City Council will retain your information for 7 years after you cease to use the service. Information is then securely disposed.

Your Rights

You have the right to:

  1. Ask to see the personal information we hold about you
  2. Ask us to change information we hold about you if it is wrong
  3. Ask us to limit the way we use your personal Information
  4. Data portability (have your data transferred to another Authority)
  5. Complain to the Information Commissioner’s Office

Withdrawal of Consent

Where personal data is processed on the basis of consent, you will have the right to withdraw that consent. Consent is obtained for the use of photography, personal stories and case studies for promotional purpose. However, Coventry city Council also process personal data that is necessary to do so to comply with our legal obligations and to enable us to perform tasks carried out in the public interest.

Complaints

You have the right to submit a complaint if you are unhappy with the way your request is handled or disagree with a decision made by the council regarding your data. In these circumstances you can contact the Data Protection Team (DPO) and request a review of the decision. DPOTeam@coventry.gov.uk
